For many health conditions, setting desired outcomes and goals is relatively easy. For example, most \u2013 if not all \u2013 people would agree that malaria is a bad health outcome. This value judgement is reflected in the <a href=\"https:\/\/www.who.int\/activities\/eliminating-malaria\" target=\"_blank\" rel=\"noreferrer noopener\">World Health Organization\u2019s goal<\/a> of \u201ca world free of malaria.\u201d&nbsp;<\/p>\n\n\n\n<p>In sexual and reproductive health (SRH) the desired outcomes and corresponding goals are less clear. The global family planning community has traditionally focused on measuring contraceptive uptake and use as measures of program success, but this raises questions: When is contraceptive use a good outcome? What if someone chooses not to use contraception?&nbsp;<\/p>\n\n\n\n<p>Unlike malaria, the answers to these questions depend on what people want.\u00a0\u00a0<\/p>\n\n\n\n<p>Family planning should be about meeting people\u2019s self-defined needs and desires when it comes to their bodies, their families, and their futures. We know that all people should have access to comprehensive SRH services, including contraception. But that does not mean that all people should be using contraception. In fact, safeguarding people\u2019s rights to switch to another method or stop using contraception altogether is equally important. These rights are not currently reflected in the way that we \u2013 as a global health community \u2013 typically measure family planning programs\u2019 success. <strong>That is why <\/strong><a href=\"https:\/\/www.psi.org\/fr\/social-business\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Viya Health<\/strong><\/a><strong> is testing out a rights-based approach to monitoring and evaluation.<\/strong>\u00a0\u00a0<\/p>\n\n\n\n<p>Viya Health, a PSI Social Business, is transforming lives in lower- and middle-income countries by ensuring equitable access to healthcare for women and girls and their partners in a digitally-enabled health and wellness ecosystem that provides comprehensive support throughout their sexual wellness journey. Viya Health is designed to support people throughout their lifetime, by offering a go-to place for affordable and high-quality information, products and services tailored to different life stages.&nbsp;&nbsp;<\/p>\n\n\n\n<p>From its conception, Viya Health\u2019s theory of change has focused on improving SRH with an emphasis on rights and autonomy. We believe that Viya Health\u2019s success should not just be defined by the number of contraceptive products it delivers, but rather by how we support people to understand their SRH rights, facilitate access to a full spectrum of high-quality, person-centered services, and \u2013 ultimately \u2013 help them live happier, healthier lives.\u00a0<\/p>\n\n\n\n<p>To ensure that Viya Health remains accountable to its rights-based theory of change, we have the following \u201ccore\u201d indicators for measuring its social impact that center person-powered SRH and autonomy.&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/gh.bmj.com\/content\/9\/5\/e013902\">Preference-aligned fertility management\u00a0<\/a><\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Person-centeredness of sexual and reproductive healthcare&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Sexual and reproductive self-efficacy&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Comprehensive sexual and reproductive knowledge&nbsp;<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p>The core rights-based indicators were pilot-tested by VIYA, Viya Health\u2019s consumer-facing brand, in their first client experience survey, fielded in South Africa in December of 2022. The survey was targeted through social media channels to VIYA\u2019s female clients, who completed the survey at a single time point after engagement with VIYA. 571 women completed the survey and most (82 percent) had engaged with VIYA by following the brand\u2019s social platforms. Respondents were primarily young adults (18-29 years old), single (50 percent), and had at least one child (74 percent).&nbsp;&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-resized\"><a href=\"https:\/\/media.psi.org\/wp-content\/uploads\/2024\/07\/12105313\/VIYA.png\"><img fetchpriority=\"high\" decoding=\"async\" width=\"755\" height=\"360\" src=\"https:\/\/media.psi.org\/wp-content\/uploads\/2024\/07\/12105313\/VIYA.png\" alt=\"\" class=\"wp-image-62546\" style=\"width:912px;height:auto\" srcset=\"https:\/\/media.psi.org\/wp-content\/uploads\/2024\/07\/12105313\/VIYA.png 755w, https:\/\/media.psi.org\/wp-content\/uploads\/2024\/07\/12105313\/VIYA-300x143.png 300w, https:\/\/media.psi.org\/wp-content\/uploads\/2024\/07\/12105313\/VIYA-18x9.png 18w\" sizes=\"(max-width: 755px) 100vw, 755px\" \/><\/a><figcaption class=\"wp-element-caption\"><em>VIYA\u2019s theory of change, including core social impact indicators.<\/em><\/figcaption><\/figure>\n\n\n\n<p>Here is what we learned:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>The majority of respondents were practicing contraceptive use in line with their preferences, but over a quarter of women were not. <\/strong>73 percent of respondents were fulfilling their self-identified contraception preferences by using or not using a method. 20 percent of respondents reported wanting to use contraception while not currently doing so, while another 6 percent reported currently using contraception despite not wanting to.\u00a0<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Our sample of women who engage with Viya had high levels of contraceptive self-efficacy. <\/strong>69 percent of respondents not currently using contraception reported confidence that they could start using contraception if and when they wanted. Among current contraceptive users, 68 and 71 percent, respectively, were confident that they could stop or switch to another method if and when they wanted.&nbsp;&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Women surveyed reported that they felt more positive about their SRH after engaging with VIYA. <\/strong>They were significantly more likely to strongly agree with statements such as <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32690468\/\">\u201cI expect to enjoy sex,\u201d<\/a> \u201cmy sexual needs or desires are important,\u201d and \u201cI know my body well\u201d when asked how they felt after engaging with VIYA versus how they felt before. 66 percent of women reported that they strongly agreed with the statement \u201cI am worthy of love\u201d after interacting with VIYA compared to 44 percent before.\u00a0\u00a0<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p>Finding high levels of contraceptive autonomy and self-efficacy \u2013 and self-reported increases after VIYA engagement \u2013 are encouraging, but we know that gaps remain and VIYA is up to the challenge of addressing them. With our core indicators centered on people\u2019s rights, we can better design interventions that reflect people\u2019s own SRH preferences and self-identified needs and that center bodily autonomy and rights.&nbsp;<\/p>\n\n\n\n<p>Want to learn more about Viya Health?
